This is new in 3.5 (you need openbox 3.5.0-3 with imlib2 support compiled in, however). Try this: http://openbox.org/wiki/Help:Configuration#Menu

Yep. Also, Openbox package in community repository already have imlib2 support compiled in since version 3.5.0-2, so if you're using that or a later version, you're good to go.
Another good tutorial on how to add icons to menu is this blog post by Og Maciel:

Regarding the conky config file, it's a mess right now. Let me clean it up first and I'll post it up for you, okay?

Just one question ... is it possible so set the undecorate property as default, instead of keybind in Openbox?

Solved the issue first applying Unia's openbox theme and then setting decor to no on rc.xml file.
