Installing and configuring Apache, PHP, and MySQL [RMx]

Under the current package scheme, there are a couple of changes to be made.

- php.ini is now in /etc/php.ini
- step two in configure php is no longer necessary, they are uncommented by default.

Otherwise, this is still a perfect wiki.

This document has been moved to the wiki. I think I did update the php.ini location, but thanks for the tip on those PHP lines being uncommented now.
