I followed the instructions for setting up b43 in the wiki, but it doesn't work.

that is a bug in kernel. i've reported upstream here: http://bugzilla.kernel.org/show_bug.cgi?id=13521
the only fix that i found is to enable the card(laptops have a button for that) and reboot. if i boot in my system with the card disabled is making the same as you.

I boot my system with the card enabled and network manager still says wireless is disabled.

now i've notice that you still load ndiswrapper which is WRONG. i think this is the main reason is not working for you. so remove ndsiwrapper from MODULES array
