I've tried a few themes along the way but always wind up going back to the default theme.
As for extensions, the forecastfox extension you mentioned is a good one for sure, and I do install it on occasion. Truthfully though, I haven't been all that impressed with most of the extensions out there.
Still, I will be keeping an eye on both themes and extensions in case something really good comes out.

I use All-in-One Gestures, Tab Mix, Spellbound and Swift tabs, which like phrakture said is awesome. But neither swift tabs nor Hit-a-Hint is the greatest Firefox extension. That would have to be Abe Vigoda Status.

None whatsoever I only use an edited chrome/userContent.css file to block ads, but I've just noticed that it's a bit outdated (/*Block Ads v.2003-08-04*/ ).
I've used quite some extensions before, but I decided I wanted to try Firefox "as it is" for a change. The only option I miss nowadays, is the automatic selection of the previously focused tab if you close one.
